Insurance and Fee Schedule


It is a good idea to familiarize yourself with your mental health insurance coverage prior to treatment.

Questions you may wish to ask your insurance company prior to your first visit:

1. Is Kimberly Butler, PMHNP an in-network or an out-of-network provider?
2. What are my out-of-network benefits?
3. Is there a deductible (amount that is paid out of pocket before benefits begin)?
4. What is my co-pay for visits with my provider?
5. Do I need pre-authorization for treatment?  If yes, from whom?
6. How many visits do my benefits cover?

In-Network Clients

As a courtesy we will bill your insurance.  You will be responsible for any fees not covered by your insurance. 

 

Out of Network Clients

We will provide you with a "superbill" to submit to your insurance for reimbursement. 

 

Cash Pay Clients

We accept clients without insurance at the rates listed on our Fee/Cash Rate Section

 

Fee Schedule & Payment


Health care coding is based on many different variables, including length of visit, level of complexity, and type of service being provided.  It often involves a combination of various codes.  How much you pay will also depend on your insurance plan (if you are using one) and the reimbursement rates that are stipulated in the contract.  So that you might budget accordingly, we have provided a list of commonly used codes and associated fees.  However, we do not know in advance which specific codes will be appropriate for your visit.  Please anticipate that if you see a provider on an ongoing basis you will receive a range of charges over time.  

We know that this can start to feel quite complicated, so if questions arise about charges, or if you’d like help understanding your insurance plan and benefits, please give Julie, (billing and insurance) a call at 541-382-1395 and she can walk you through it.

There are several services that we elect not to charge for: the occasional short phone call with client, coordination of care with client’s other providers, an occasional special document requiring less than 10 minutes.
