Validation Code Portal Option

You will need a 4-digit code (generated by office staff or your provider at your office visit) The code is only good for 7 days-respond promptly for best results.

In addition to receiving the 4-Digit Validation Code (i.e. 3148) you will also be sent an email invitation from no-reply@valant.com

1.     Click on the link within the email invitation to be directed to the Patient Portal for the practice https://valantmed.com/portal/kimberlybutlerpmhnp

2.     Enter your Last Name, the 4-Digit Validation Code when prompted

3.     Type the security code

4.     If your 4-Digit Validation Code has expired, you can request a new code at your next office visit.

5.     Click on the Setup Account button

6.     You will then be prompted to create your own username, password, and security questions.

7.     Click on Create Account

8.     Your account setup is now complete, and you will be directed to a confirmation page.
