Scheduling Process

We are a referral only practice. Please have your therapist or PCP send your information to the office attention Kim Butler, PMHNP via secure fax 541-382-6576.

Therapist Referral: Please have your therapist or counselor send their initial intake note and most recent progress notes. Most therapists will require you to sign a release of information. You can find the form here: Release of Information

PCP Referral: Please request a referral from their office (can be done over the phone, or you may need to be seen in person depending on your insurance carrier). Your PCP most likely has a referral department or staff who specifically takes care of these requests.

Medication Refills, Clinical/Medical Needs or Crisis:

Prescription Refills: Patients are provided enough medication refills at their follow up visits to last until their next appointment.  If you need a refill prior to your next appointment, please call your pharmacy. Allow 3 - 7 business days for turnaround time

If you are taking a controlled substance, such as a stimulant for ADHD or a benzodiazepine for anxiety or sleep, you need to be seen for follow up every 3 months for your provider to remain in compliance with DEA (Drug Enforcement Agency) guidelines.

Clinical/Medical Needs: Please call the office 541-382-1395 and follow the prompts to leave a brief message. Most calls left during business days (Monday-Thursday) are returned by the next business day. Non-urgent calls left on Friday, Saturday or Sunday will be returned on Monday.

If you are in CRISIS: Please go to the nearest emergency room or call 911.

The Deschutes County Crisis Stabilization Center is available for walk-in 24/7 365-days per year.
